2021 Benefits Open Enrollment runs from 12/01/2020 - 12/22/2020


BEFORE YOU BEGIN, PLEASE BE SURE YOU HAVE THE FOLLOWING:

YOUR DATE OF HIRE AND JOB TITLE


SSN, DATE OF BIRTH AND ADDRESSING OF ALL ENROLLING
IF APPLICABLE, MEDICARE ID NUMBER AND EFFECTIVE DATE, CURRENT CARRIER NAME, POLICY NUMBER AND START DATE, PRIMARY CARE PHYSICIAN INFORMATION.

Please review this checklist prior to completing your Benefit Elections/Changes.

NOTE: If supporting documents are required for your election/change, AND NO DOCUMENTION IS RECEIVED WITHIN 7 DAYS IN THE BENEFITS OFFICE, YOUR ELECTION/CHANGE WILL NOT BE APPROVED!


For Medical and/or Dental Enrollment:

If enrolling a spouse – a copy of your marriage certificate must be provided within 7 days of your election.

If enrolling a domestic partner – a copy of State of California Certificate of Registration of Domestic Partnership must be provided within 7 days of your election.

TO ENROLL, OR WAIVE MEDICAL, DENTAL, VISION AND

LIFE/ADD BENEFITS COVERAGE

Keep an eye out for Open Enrollment Information arriving by Email from noreply@ease.com at the end of November. Your email will provide instructions for logging in and Electing or Waiving Coverage.



If you are Electing Coverage, you will have the option to Elect Pre-Tax or Post-Tax deduction.

* All employees must update and complete 2021 PGD EE INFO SHEET come the New Year. PGD requires an updated form regardless if the information has not changed from previous years.

OPTIONAL:


The 2021 IRS Form W-4 has not been released yet. Please check back - It will be posted as soon as it has been released.

* Employees who elect to change their withholdings on January 1, 2021 are required to submit a new 2021 IRS Form W-4. Employees who have submitted Form W-4 in any year before 2021 and not required to submit a new form. Employers will continue to compute withholding based on the information from the employee's most recently submitted Form W-4.
